EECS W26 Deferred Standing Application Form

Please complete the below form if you have missed or will miss an EECS W26 final exam during the official exam period due to illness or unforeseen emergency.

If you are a student requiring religious accommodation for an exam, if you have not already done so, please email eecs-ug@yorku.ca for instructions.

Please complete this form once for each EECS course you have missed or will miss an exam for.

You are required to complete this form within 5 business days of the exam you have missed.

You are required to attach evidence to explain your absence, in alignment with Senate policy. You will not be able to submit this form without attaching at least one piece of evidence.

Students who are approved to attend the deferred exam on May 1 will be contacted by April 28.

Courses with a lab exam are listed on this form. After you submit your entry for a lab exam course, your name will be passed to your course director and the course director is responsible for scheduling your deferred lab exam.

In all cases of illness, a York University Attending Physician's Statement is mandatory.

Any file you attach must clearly evidence why you were unable to attend the original exam as scheduled.
